Application Process
Act now to be a part of PCCA’s 36th year!
Your child’s application is considered complete and ready to process when the school has received the following items. The Admissions Office must receive the following before testing or interviews will be scheduled:
For Preschool & Kindergarten Applicants:
- Application Fee ($300 per family, non-refundable)
- New Student Application 2018-2019
- Copy of certified birth certificate
- Principal Recommendation Form (give to Day Care or Preschool director if applicable)
- PK3-K5 Teacher Recommendation Form (the parent should complete if student is not currently in a daycare or preschool program)
- Student Emergency Medical Form 18-19
*Principal and Teacher Reference Forms should be completed by those individuals and returned directly to the school by mail or fax.
For 1st – 5th Grade Applicants:
- Application Fee ($300 per family, non-refundable)
- New Student Application 2018-2019
- Copy of certified birth certificate
- Most recent standardized test results (if applicable)
- Previous report cards (last full year and current school year)
- IEP or 504 Plan, if applicable
- Principal Recommendation Form
- Elementary Teacher Recommendation Form
- Student Emergency Medical Form 18-19

For 6th – 12th Grade Applicants:
- Application Fee ($300 per family, non-refundable)
- New Student Application 2018-2019
- Copy of certified birth certificate
- Most recent standardized test results (if applicable)
- Previous report cards (last full year and current school year)
- Completed transcript (for students entering grades 11 or 12)
- IEP or 504 Plan, if applicable
- Principal Recommendation Form
- Math Teacher Recommendation Form
- English Teacher Recommendation Form
- Student Emergency Medical Form 18-19

The Application Process continues as outlined below:
Testing:
Placement testing is scheduled for all students applying as determined by the principal. There are no additional fees for testing (with the exception of additional testing for the Discovery program, if required).
Interview:
After the above items are submitted to PCCA, you will be contacted to schedule a family interview with School Administration.
Admissions Committee:
After completion of all of the above, your application will be reviewed by our Admissions Committee. You will be notified in writing as to your child’s acceptance. If your child meets the admissions requirements but there are no spaces currently available in that grade, you will be given an opportunity to keep your application active in the waiting pool.
Enrollment:
With your acceptance letter you may be sent forms which must be completed and returned, with applicable fees, to the Admissions Office before your child is officially enrolled.
