If your son or daughter is thinking about playing a sport for PCCA please make sure that you have the FHSAA Physical form completed by their physician before the start of the school year.  Sports physicals are only valid for one year and must be renewed annually.    Also required is a current FHSAA Consent form to be on file for all sports participants.  Both of these documents can be downloaded through the links below.

For new students, please be reminded that the FHSAA physical can be completed as the required health physical for all new enrolling students.
